Required Sports Forms

All Sports forms are available for completion in ParentSquare. The forms that must be completed are the Co-Curricular Code of Conduct, the Medical Consent form, and the Concussion form. Additionally, all athletes need a physical on file in the school office in order to participate in their respective WIAA sports. Intramural 6th grade girls volleyball and 6th grade flag football are exempt from the physical requirement but must still complete all other forms.

Athletic/Club Required Form Locations
Required forms to participate in Athletics and/or Clubs for students in grades 6 – 12 are found in ParentSquare. The completion of these forms is REQUIRED before your student can attend practice for their sport.
If additional help or a device to access the forms is needed, please feel free to stop in the WRAMS main office for assistance any time that school is in session.
